To OP, I'm not sure how long you've been dating jake or how serious things became between you two but it's clear from reading what you wrote that things got serious enough that you still care about him. I think the best thing for you to do is to call him back and just the two of you talk about everything; your relationship, where you two see things going forward, what happened with your kids, why he did what he did. Once you have a good talk with jake I'm sure you'll have a better idea of where you need to take things and will feel a bit more confident about your decision to continue or end your relationship with him.
