I had this also....and ended up getting circumcised. It was my choice for reasons of sanitation. I simply could not keep the underlip of my mushroom head clean because I was not getting everything out from under it when I showered. I used cue tips with Balneol to clean it out but that got really old. After I gave my wife a bad case of cystitis a couple of times, I had it examined by two different doctors. Both said they could cut the skin and stitch it...but it would be more painful than a circumcision.....so I went for the circumcision. It was a breeze and was not painful until I got a hard on in my sleep while healing. After the circumcision I found that my cockhead mushroom was free at last and has actually grown some especially just before I cum....and my sex partners are saying "WOW" when I use it to pop in and out of them to tease their clit....they are lovin it, and so am I.
The guy that said "See a plastic surgeon" was right if you want it repaired. Otherwise for hygiene sake, get the circumcision. You may not realize what you are missing (LOL)
